Intake Forms

Please complete all forms through our online scheduling site prior to your first appointment. The information you provide is important for your PT to get a good understanding of why your are seeking physical therapy and early completion allows for best use of time at your initial evaluation.

What to Expect

Evaluations and treatments at Balance+Flow are always one on one with  our licensed physical therapists. The initial assessment will include a thorough intake and medical history for the issue that brought you in, review any previous studies or reports, and understand your expectations & goals.

A physical examination will also be completed which will likely involve movements of the spine and extremities both actively, passively and functionally. The therapist will perform any necessary special tests to determine which specific portion of a joint, ligament, tendon, and/or muscle appears most effected or diminished.
The information gathered will help the PT begin to develop a plan specific to your individual needs. Explanation of findings and diagnosis will be discussed and then specific activities/exercises will be issued to complete at home prior to your next visit. 
Depending on the complexity of the issue that brings you in, sometimes the initial visit is more consultation than treatment. Understanding and hearing your story is paramount to us in order to develop the right PT program for you.
Visits are typically 45-60 minutes in a private, calm environment. 

What to Bring


 Please bring yourself and the following items early to your first appointment. We pride ourselves on staying on schedule and fully using the short amount of time we have with each client.  Late arrive will compromise our ability to provide the best experience possible.
For you first visit please bring the following as they apply to you:
  • Physician reports, diagnostic tests (MRI, Xrays, Sleep Study, etc)​
  • Medication list
  • List of other providers on your care team
  • Comfortable clothing you are able to move in
  • Shoes you wear commonly for daily wear and exercise
  • Braces or oral appliance you are currently using.
